Health Form AB

Health form for local trips and trip under 72 hours. This form applies to Scouts and Leaders. If Scouts/Leaders intend to go to any long term camp (ie, summer camp, Jamborees or any of the High Adventure Bases) the Class ABC form is needed.

One of the forms, completed is required yearly for all registered Scouts or Leaders.
