Health Form ABC

Health form for long term trips and High Adventure Bases. This form applies to Scouts and Leaders. If Scouts/Leaders intend to go to any long term camp (ie, summer camp, Jamborees or any of the High Adventure Bases) the Class ABC form is needed. This single form is sufficient for all the High Adventure Bases (each High Adventure Base does not have a unique form).

Either of the forms (class AB or ABC), completed is required yearly for all registered Scouts or Leaders.
